By registering for Foursquare Leader Conference (FLC), I agree that my image may be used by The Foursquare Church in event-related materials. By submitting this registration form, I agree to the cancellation policy stated below. By registering for FLC, I agree to receive periodic emails from The Foursquare Church. I will have the option to opt-out at any time; my information will not be shared with a third party.

If you or anyone in your household has been exposed to COVID-19 in the 14 days preceding the start of FLC, even if not exhibiting symptoms, you may not attend. While we encourage our attendees to err on the side of caution, we will not be conducting any COVID-19 tests nor checking vaccination status. All attendees will be required to abide by the COVID-19 event protocols that are set in place at the time of FLC. While we will be taking precautions and following local safety requirements that are in place at the time of the event, we cannot be held responsible if you contract the COVID-19 virus while attending FLC. It is widely known that COVID-19 is highly contagious and cannot actually be contained. By registering for FLC, you assume the risk related to COVID-19 transmission for yourself and any minors under your control, and you hereby are releasing and agree to hold The Foursquare Church (aka International Church of the Foursquare Gospel) harmless from any damages, losses, claims or injuries, whether to your person, your family or your property, occurring while attending FLC, unless caused by the gross negligence or willful misconduct of Foursquare. If you contract COVID-19 during the event, registrations are non-refundable.

Requests for registration refund or cancellation must be received in writing via email (bjohnstone@foursquare.org) at least 48 hours before the event start time.

• Refunds or cancellations may be subject to a $35 processing fee.
• Phone cancellations will not be accepted.
• Registrations are non-transferable.

If the event is canceled, registration expenses will be refunded. Travel expenses are the attendee’s responsibility and will not be compensated by The Foursquare Church.
