We and our employees want to make contributions toward retirement; how do we do this?
A Monthly Data Report must be submitted to Foursquare Retirement Services for all participants who elect to have deductions from payroll, and/or for churches that elect to make contributions on the behalf of their employees. Click below for instructions on how to format your Monthly Data Report and church checks:

- Plan Enrollment Form (1.1 MB, 3 pages)
Use this form to make changes to address, beneficiary or investment allocations. If you have never participated in the Plan, contact Foursquare Retirement Services for the full Enrollment Kit.
- Spousal Waiver of Benefit Form (172 KB, 2 pages)
Use this form if your spouse is to receive less than 100% as your beneficiary.
- Salary Reduction Agreement (132 KB, 1 page)
Use this form to advise your payroll supervisor of an elective deferral--do not submit this form to Foursquare or Lincoln Financial Group. Print the Monthly Data Report and Check (PDF, 360 KB, 3 pages) instructions and contact Foursquare Retirement Services if you have questions on the procedures for remitting and reporting your payroll deductions.
- Rollover/Transfer Request (208 KB, 4 pages)
Use this form to rollover or transfer eligible funds into the Foursquare Retirement Plan. Before you complete the forms, contact Foursquare Retirement Services to determine that the funds are eligible to rollover/transfer under current tax laws and Plan provisions.
