To comply with the requirements to maintain certification, each Foursquare Ministry Training institute must submit an Annual Report by August 1 each year. To submit your report, please download, complete and submit this form as per the instructions at the bottom of this page.
All Foursquare Ministry Training Institutes are asked to inform the Institutes Department of any substantive changes that occur in their institute. A substantive change is any major change in the constitution or program of an institute. This includes changes in governance, curriculum, administration, location or any other significant change. To submit notice of a substantive change, please download, complete and submit this form as per the instructions at the bottom of this page.
In addition to an annual report, each institute is asked to submit a student profile card for each of their students. You can download and print additional copies of these cards as needed.
